The main goal of this initiative is to increase the autonomy of state medical institutions to expand their managerial and financial powers, which will allow for quick decisions on the implementation of modern management methods and the improvement of paid services to optimize budget expenditures.
According to the information from the justification note, state medical institutions currently face a number of systemic problems, including:
- insufficient financial and managerial independence, especially in personnel matters;
- limited opportunities for attracting additional resources and implementing innovative management methods.
As part of the pilot project, a temporary procedure will be implemented that will allow the pooling of financial resources into a common fund with subsequent redistribution of funds according to priority areas. Institutions will also have the right to independently establish their organizational structure and staffing schedule based on the number of patients and the list of services offered.
This will allow for a more effective adaptation of the work of medical institutions to the real needs of the population and improve the quality of medical services.Particular attention is also paid to financial autonomy. In particular, it is proposed to retain unused balances of funds for medical organizations at the end of the financial year, which will contribute to the optimization of expenditures and the creation of reserve funds for the renewal and acquisition of high-tech equipment.
In addition, the project includes a new provision on remuneration and financial incentives. It retains guarantees for employees but proposes to abandon the formal approach based solely on position or salary scale. The new remuneration system will take into account work performance and provide managers with the opportunity to set salary levels considering minimum guaranteed payments.Regulations on remuneration and financial incentives for healthcare organization employees in a pilot mode.
An important point is that the work of state medical organizations within the trial mode will be carried out within the funding allocated for 2026, without reducing the volume and quality of medical services under the State Guarantee Program.
Additional resources are planned to be attracted through donors and sponsors. For this purpose, medical institutions will be allowed to open accounts at OJSC "Eldik Bank".
Another significant innovation will be the development of mechanisms for setting prices for medical and other services. These mechanisms will not be subject to the legislation on state and municipal services for those organizations participating in the trial. The Ministry of Health, together with the Ministry of Economy and Commerce, has been tasked with developing and approving a methodology for setting tariffs for paid services, with mandatory consideration of benefits for socially vulnerable groups of the population.
Leaders of medical organizations will be responsible for calculating and approving prices for services according to an agreed methodology. As stated in the document, this should contribute to the creation of a competitive environment, increase financial revenues, and enhance the motivation of medical workers.
The developers emphasize that the new methodology will not lead to unjustified price increases but will ensure fair pricing considering inflation, utility costs, and other factors. Currently, the cost of paid services in state medical institutions, according to the Ministry of Health, is 2-3 times lower than in private clinics.
The Ministry of Health is confident that the comprehensive implementation of the proposed measures will increase the interest of managers in work results, strengthen the motivation of medical workers, and improve the quality of medical care as well as patient satisfaction levels. Upon completion of the trial, an analysis will be conducted, and a report with proposals for further reforming the healthcare system will be prepared.
