Project Goals
Erkin Checheybaev, who previously held the position of Minister of Health, has already expressed his intention to ensure the financial autonomy of state hospitals. In an interview with Densooluk.media, he raised the issue of legalizing paid medical services: "Our task is to allow healthcare institutions, especially hospitals, to earn legally based on self-financing. All financial flows that exist are not directed towards the development of institutions but go to individual doctors. We need to bring these funds out of the shadows," he noted.Checheybaev also added that there is a possibility since medical institutions have special accounts and price lists for paid services.
Later, the Ministry of Health clarified how state hospitals will be able to earn legally. "Healthcare organizations will have the opportunity to generate funds for the modernization of their material and technical base and to increase salaries. One example could be the introduction of services such as hair transplants, which go beyond the state guarantee program," noted Elaman Charginbaev, head of the planning and financing department of the Ministry of Health.
Regulatory Document
By the decree of President Sadyr Japarov, a pilot project for granting autonomy to medical institutions is being launched.According to the decree dated November 28, 2025, No. 31 "On Measures to Enhance the Potential and Competitiveness of State Healthcare Organizations of the Kyrgyz Republic," there is a need to test new management approaches in the context of systemic problems in state hospitals. It notes that the existing management model limits the institutions' ability to effectively use their revenues.
In order to improve the quality of medical care and create conditions for the successful functioning of institutions, the decree introduces a trial regime from January 1 to December 31, 2026.
The Cabinet of Ministers is tasked with determining the list of organizations for which this regime will be implemented, as well as approving a temporary procedure for their regulation.
Powers of Medical Institutions
According to sources on the portal economist.kg, the hospitals and other state medical organizations participating in the project will receive the following rights:- to independently develop their organizational structure;
- to determine the number of employees in accordance with new standards;
- to establish forms and amounts of remuneration;
- to manage their own income and resources obtained as a result of optimization;
- to open accounts in "Eldik Bank" for payments for paid services;
- to develop and approve tariffs for paid services in the manner established by the Ministry of Health, while maintaining preferential tariffs for vulnerable groups of the population.
Number of Participants
Kanibek Dosmambetov reported that about 20 medical institutions will participate in the pilot program, which will receive expanded powers.If the experiment proves effective, it may be scaled up at the republican level.
Model to Follow
The management model used in the medical center of Osh State University will be taken as a model for the implementation of this project."When services are provided at official tariffs, funds go to the institution rather than disappearing into the shadows. This allows for at least 50% of revenues to be directed towards increasing the salaries of medical staff and the development of institutions. This approach significantly reduces the level of corruption and creates a sustainable motivation system," noted Kanibek Dosmambetov.
