According to regulations, the title may be awarded for:
- long-term and effective charity and patronage;
- acts demonstrating courage in the interests of the residents of the capital;
- significant achievements in various fields that have gained recognition among the population, including state, municipal, political, scientific, cultural, and social areas that benefit Kyrgyzstan and Bishkek;
- long-term fame in the urban environment, which must be at least 15 years, as well as for those who have held the positions of mayor, chairman of the Bishkek city council, or deputies elected for at least three terms.
Proposals for awarding the title may come from BGK deputies, the mayor's office, local self-government bodies, public organizations, and individual citizens in the form of official appeals.
The awarding of honorary titles takes place at the annual session of the BGK, timed to coincide with City Day.
In 2025, the title of "Honorary Citizen of Bishkek" was awarded to 13 citizens.
